HealthReach offers the following medical services at the following times each week.  All services are by appointment only.

Family-Practice Style Medical Care           Thursday              5:00 PM - 9:00 PM

Chronic Disease Clinic                               Saturday              9:00 AM - 12:00 PM

                                                                   Tuesday               9:00 AM - 9:00 PM

Lab services are scheduled during the above hours. We also offer gastro-intestinal care, neurological care, and limited OB/GYN services by referral only.

New patients are seen on Thursdays from 5:00 PM - 9:00 PM only. Please see Eligibility for information on who is eligible and documents that must be brought for a first appointment.

IMPORTANT NOTE: AFTER SEPTEMBER 13, 2010 NEW PATIENTS MUST BE SCREENED FOR ELIGIBILITY BEFORE AN APOINTMENT CAN BE SCHEDULED. APPLICANTS CAN DROP IN BETWEEN 12:30 PM AND 3:00 PM ON TUESDAYS AND FRIDAYS FOR INCOME SCREENING.
