To be eligible for services at HealthReach Community Clinic, you must
- Live in Mooresville, Troutman, or Mt Mourne
- Make less than 250% of the Federal Poverty Level
- Have no medical insurance, including Medicaid/Medicare
Patients must pre-register before making an appointment. To register, patients should come to the clinic between 12:30PM-3:00PM on Tuesdays, Thursdays, and Fridays, and bring the following:
- A government issued photo ID (Driver's license, Passport, foreign goverment ID)
- Proof of address in Mooresville, Troutman, or Mt Mourne (utility bill, lease agreement)
- Documents showing income for all members of the household (paystubs, unemployment, disability, social security, child support, etc.)
