• Be a resident of Mooresville, Troutman, or Mt Mourne
• Make less than 200% of the Federal Poverty Level
• Have no insurance whatsoever, including Medicaid/Medicare

You are required to bring the following with you to your first visit at HRCC: 

• A picture ID
• Proof of residency (Drivers license or utility bill addressed to YOU at a Mooresville, Troutman, or Mt Mourne address)
• Proof of one month's income for ALL members of the household(pay stubs, copies of paychecks, a monthly bank statement, proof of unemployment or disability)
• A list of all medications you are currently taking

IMPORTANT NOTE: AFTER SEPTEMBER 13, 2010 NEW PATIENTS MUST BE SCREENED FOR ELIGIBILITY BEFORE AN APOINTMENT CAN BE SCHEDULED. APPLICANTS CAN DROP IN BETWEEN 12:30PM AND 3:00 PM ON TUESDAYS AND FRIDAYS FOR INCOME SCREENING.
