The Chronic Disease Clinic is available every Tuesday from 10:00 to 9:00 and Saturday from 10:00-12:00 by appointment only. Clients who have been seen in at our regular medical clinic and have been diagnosed by our doctors as having a chronic illness (hypertension, diabetes, etc.) can be enrolled into this clinic. Patients in this clinic are seen on a regular basis by the same physician. This allows us to provide continuity of care, and monitor the patient’s progress over time.
